Caregiver Checklist: Reviewing the PT Section of an IEP

This checklist is for caregivers who want to understand whether the physical therapy portion of an IEP is clear, functional, and connected to the student’s school day.

It is not a substitute for legal advice, clinical evaluation, or individualized advocacy. It is a starting point for asking better questions.

Use this checklist before or after an IEP meeting to notice whether the PT-related parts of the plan explain the student’s needs, the support being provided, and how that support connects to educational participation.
