What Should I Ask the Team to Put in Writing?

If a concern, recommendation, service change, or decision matters, it is reasonable to ask where it will be documented.

This does not mean every conversation needs to become formal or adversarial. It means that important decisions should be clear enough that caregivers, staff, and future team members can understand what was discussed, what was decided, and what happens next.
